Jay Moore, MD

Jay Moore, MD (he/him) is board certified in internal medicine, and keeps up on all the latest clinical information in order to provide evidence-based medicine. He is a provider for Nexus Primary Care in Brentwood and offers care for those 13+.

Gender-affirming Care: Dr. Moore believes in the rights of trans people to live their lives, and he'll support and affirm you no matter where you are in your journey. He is happy to write letters, coordinate referrals, prescribe medications, and do whatever you need so you can live your best and most authentic life. Dr. Jay has worked with the LGBTQIA+ Community for more than ten years through volunteer work, professional organizations, and community outreach.

Cost: Nexus is a Direct Primary Care practice, which means they accept a monthly subscription fee of $150/adult to give patients unlimited access to Dr. Moore and the office. This includes unlimited visits, up to an hour each. There’s never a fee or co-pay for seeing Dr. Moore and you may call or text him whenever you need. A few in-office procedures are offered at no charge as well, like EKGs, earwax removal, steroid injections of knees and other painful spots, abscess drainage, and stitches for minor cuts.

Medication: Can prescribe ADHD meds, pain meds, hormones, antipsychotics, etc.
