How to pay:

• Offers insurance assistance with enrollment specialists trained to educate and enroll patients in available health insurance plans that will accommodate the individual’s healthcare and financial needs.
• At least $35 per visit.
• Offers an income based sliding scale for patients without health insurance who also don't qualify for Medicaid. Patients are required to meet the income eligibility by submitting proof of household income.
