Linkage to Care (Formerly HEAT)

Description

Linkage to Care (LTC), formerly called HEAT (HIV Engagement Adherence Team) provides intensive case management specifically for people who are newly-diagnosed with HIV within the last 12 months.

LTC works closely with newly-diagnosed people to help them understand their HIV diagnosis, get them scheduled to see an HIV primary care doctor, and make referrals for needed services. LTC is a time-limited service, but persons who continue to need support may transition to Ryan White case management.

  • Linkage to Care is a free service, provided by social workers and other skilled professionals
  • Eligibility requirements are the same as for Ryan White case management
